Introduzione Dalla ricerca bibliografica si evince l’importanza di una dimensione educativa propria dell’assistenza infermieristica nei confronti dei pazienti prossimi alla dimissione dall’ospedale verso il proprio setting quotidiano. La Direzione infermieristica dell’Azienda USL di Reggio Emilia, nell’ottica di ampliamento delle competenze professionali, ha promosso un progetto di miglioramento relativo all’empowerment di infermieri e pazienti relativamente alla dimissione dal contesto chirurgico. Obiettivo Questa tesi indaga i bisogni espressi dal paziente dimesso da un contesto chirurgico e analizza il ruolo educativo della figura infermieristica così come descritto in letteratura. Scopo della tesi è favorire la consapevolezza dell’infermiere riguardo a tale ruolo, porlo in essere e valutare, in termini di soddisfazione dei bisogni, la ricaduta di nuovi modelli educativi, omogenei per i differenti dipartimenti chirurgici dell’Azienda Usl di Reggio Emilia, sviluppati dagli infermieri. Materiali e metodi Al fine di analizzare il bisogno educativo percepito dai pazienti dimessi dalle chirurgie dei presidi USL di Reggio Emilia, sono state loro somministrate 118 interviste telefoniche. Gli intervistatori sono stati 4 infermieri che esercitano la propria professione nelle strutture chirurgiche di degenza di ciascun paziente coinvolto. L’indagine, guidata da un questionario, ha indagato la percezione dei pazienti in termini di educazione adeguata e in un contesto consono. I dati così raccolti ed elaborati sono stati commentati, attraverso la modalità del brainstorming durante la formazione sul campo, ed approfonditi da 10 infermieri coinvolti nel progetto. A questi professionisti è stata rivolta una formazione sul campo della durata di quattro giornate, relativamente all’empowerment professionale in un contesto di educazione del paziente chirurgico alla dimissione. I professionisti hanno avuto l’occasione di apprendere basi di metodologia della ricerca e di confrontarsi sulle evidenze presentate dalla letteratura. A partire da queste, poi, hanno costruito del materiale educativo da proporre ai pazienti delle proprie realtà lavorative, in un’ottica di empowerment alla dimissione e secondo i principi di Health Literacy. Risultati A partire dalle riflessioni sul ruolo educativo dell’infermiere riscontrate in letteratura, è emersa la necessità, da parte dei partecipanti alla formazione sul campo, di valorizzare la propria professione nell’ottica di una mission educativa. Dall’analisi dei dati raccolti a partire dai questionari, per ogni quesito è evidente la disomogeneità tra le diverse strutture sia in termini di casemix che di modalità educative. Il materiale informativo in uso viene consegnato, con discontinuità e frammentarietà, ai pazienti nelle diverse realtà. Al fine di analizzare i dati raccolti è stato richiesto il contributo degli infermieri coinvolti. Ciò ha stimolato la discussione e la ricerca dei professionisti nell’esplorare nuovi approcci educativi alla dimissione. Sono state prodotte 5 brochures relative alla gestione del dolore, della ferita, a interventi in videolaparoscopia, artroscopia, e di ernia inguinale. Tale materiale è attualmente in attesa di essere validato dall’Ufficio comunicazione . Conclusioni Questo percorso ha portato gli infermieri coinvolti, attraverso una riflessione guidata, a rinnovata consapevolezza del proprio ruolo educativo. Gli stessi, inoltre, hanno avuto l’opportunità di acquisire nuove competenze di ricerca bibliografica e di Health literacy e di produrre materiale informativo relativo alla dimissione dall’area chirurgica destinato ai pazienti. In seguito all’implementazione di strumenti informativi omogenei nelle diverse unità operative dell’Azienda Usl di Reggio Emilia, potranno essere valutati l’impatto e la ricaduta di questo progetto formativo sulla soddisfazione dei bisogni espressi dai pazienti.

Abstract

From the literature, search shows the importance of its educational dimension of nursing care to patients coming to hospital discharge towards their daily setting. The Department of Nursing of the USL Reggio Emilia, in order to expand professional skills, has promoted a project to improve its empowerment of nurses and patients in relation to the discharge from the surgical context.
target
This thesis investigates the needs expressed by the patient discharged from a surgical context and analyzes the educational role of the figure nursing as described in the literature. The aim of the thesis is to promote awareness about the role of the nurse, put it in and be assessed in terms of the satisfaction of needs, the relapse of new educational models, homogeneous for the different surgical departments the Local Health Authority of Reggio Emilia, developed by nurses.
Materials and methods
In order to analyze the educational need perceived by patients discharged from the surgery principals USL Reggio Emilia, they have been given 118 telephone interviews. The interviewers were four nurses who pursue their profession in the surgical facilities of hospital stay of each patient involved. The investigation, led by a questionnaire, investigated patients' perceptions in terms of education and adequate in a context appropriate. The data thus collected and processed are discussed through the methods of brainstorming during the field training, and in-depth by 10 nurses involved in the project. These professionals has been given a field training lasting four days, relatively professional empowerment in the context of education of the surgical patient at discharge. The professionals have had the opportunity to learn the basics of research methodology and discuss the evidence presented in the literature. From these, then, they have built educational material to be offered to patients of their working situations, with a view to empowerment at discharge and according to the principles of the Health Literacy.
results
Starting from the reflections on the educational role of the nurse found in the literature, the need has arisen on the part of participants in the training field, to enhance their profession with a view to the educational mission. The analysis of data collected from the questionnaires, for each question there is an obvious lack of homogeneity between the different structures both in terms of case-mix that mode of education. The material used is delivered, with discontinuity and fragmentation, to patients in different realities. In order to analyze the collected data was required the contribution of nurses involved. This has stimulated discussion and research of professionals in exploring new approaches to education at discharge. 5 were produced brochures relating to the management of pain, injury, interventions in videolaparoscopy, arthroscopy, and inguinal hernia. This material is currently waiting to be validated by communication.
conclusions
This path has led the nurses involved, through a guided reflection, a renewed awareness of its educational role. The same also had the opportunity to acquire new skills in literature search and Health literacy and produce information material concerning discharge from surgery for patients. Following the implementation of information tools homogeneous in the different units of the Health Services Reggio Emilia, will be assessed the impact and effect of this project on training satisfaction of the needs expressed by patients.
