Abstract
Objective: report the application of fluorescence confocal microscopy (FCM) combined with Mohs microsurgery in the urological field assessing its diagnostic accuracy for non neoplastic and cancerous prostate tissue (prostatic adenocarcinoma) in surgical margin assessment. Patients and methods: In all, 36 specimens from 8 patients with prostate cancer were enrolled into the study. All patients underwent robot-assisted laparoscopic radical prostatectomy. Mohs micro sections were cut from the prostate. Specimens were analyzed using FCM and HE by two consultant pathologists. Intra- and inter-observer agreement was tested by the means of Cohen's κ. Results: The overall diagnostic agreement between FCM and histopathological diagnoses was substantial and even inter-rater reliability was perfect. Conclusion: FCM combined with Mohs microsurgery seems a promising strategy for real-time margin assessment. This approach can show with a better precision the exact location of persisting cancer, allowing more accurate secondary resections and giving the possibility to maximize the nerve sparing surgery. |
