A partire dal 1970 vengono descritti in letteratura casi di primi molari e incisivi permanenti con ipomineralizzazione idiopatica dello smalto; nel 2001 Weerheijm propone il termine MIH, molar incisor hypomineralization, per definire un difetto qualitativo dello smalto, che può interessare incisivi e primi molari permanenti.Lo smalto si presenta ipomineralizzato e opaco,dall’aspetto poroso e di colore bianco o giallo o marrone,con i bordi periferici più chiari e nettamente distinguibili dallo smalto sano.A volte il difetto è così poroso che dopo l'eruzione, il dente può andare incontro a fratture dello smalto, posteruptive enamel breakdown, sotto le forze masticatorie,lasciando la dentina esposta e favorendo così lo sviluppo di lesioni cariose. La prevalenza mondiale dei diversi difetti di MIH varia, secondo la letteratura, dal 2,4% al 40% in base alle aree geografiche; colpisce più il sesso maschile e si riscontra prevalentemente sulle superfici buccali e occlusali degli elementi interessati. I ricercatori hanno riscontrato che i bambini con MIH richiedono maggiori cure rispetto ai bambini non affetti: i denti possono risultare più sensibili a cibi ed aria fredda per la maggior porosità e la struttura alterata che comporta anche una maggior possibilità di penetrazione dei batteri attraverso lo smalto ipomineralizzato fino ai tubuli dentinali.L’accentuata ipersensibilità, nei casi più gravi, ostacola le quotidiane manovre di brushing, causa di discomfort e dolore. Il quadro patologico e l’accumulo di placca per la scarsa igiene, espongono a più rischio di carie i bambini con MHI, ulteriormente difficili da curare per la difficoltà nell’anestetizzare questi denti ipersensibilizzati da continui stimoli termici e meccanici. La scarsità dei dati presenti in letteratura sul trattamento dell’ipersensibilità ci inducono a valutare ipotesi di trattamenti alternativi che migliorino questa problematica. Ultimi dati del 2018 confermano il miglioramento dell’ipersensibilità degli elementi con MIH dopo l’uso di remineralizzanti a base di CPP-ACP rispetto a quelli a base di fluoro, confermando i dati di Ozgul che trattava però solo incisivi con MIH. Bekes riporta esiti incoraggianti dopo l’applicazione di una pasta desensibilizzante a base di arginina 8% e calcio carbonato per 8 settimane. L’obiettivo dello studio è valutare l’efficacia del trattamento di erosione-infiltrazione con resina, in molari permanenti con MIH, nell’attenuazione dell’ipersensibilità durante le manovre di igiene o l’assunzione di cibi/bevande freddi.Sono stati selezionati 21 pazienti di età tra 8 e 14 anni con sensibilità agli stimoli termici su almeno un molare permanente,valutata con scala Shiff con valori superiori a 0 prima del trattamento (T0). Gli elementi selezionati non avevano carie o fratture post eruttive.Dopo un singolo trattamento di erosione-infiltrazione con ICON, si effettua il test per la sensibilità al termine dell’applicazione (T1), a una settimana dal trattamento T2, a un mese e due mesi dopo (T3, T4).Tutti hanno riportato valori inferiori di sensibilità già al termine del trattamento con un andamento migliorativo nel tempo fino alla totale scomparsa; 4 continuano ad avere una lieve percezione dello stimolo termico a distanza di due mesi. Il trattamento di erosione infiltrazione con resina ICON è un trattamento preventivo mininvasivo che consente di occludere, grazie alla resina fluida, le porosità dello smalto affetto da MIH migliorando significativamente il problema dell’ipersensibilità e di conseguenza le manovre di igiene orale domiciliare o il discomfort durante l’assunzione di cibi e bevande troppo fredde. Un’ipotesi è che il trattamento con resina migliori verosimilmente la struttura irregolare dello smalto andando a diminuire gli stress meccanici a carico degli elementi dentali e quindi il rischio di fratture post eruttive e l’esposizione dei tubuli ai batteri.

Abstract
Starting from 1970, cases of first molars and permanent incisors are described in the literature with idiopathic hypomineralisation of the enamel; in 2001, Weerheijm proposed the term MIH, molar incisor hypomineralization, to define a qualitative defect of the enamel, which may affect incisors and first permanent molars. The enamel of the affected teeth is hypo-mineralized and opaque, with a porous appearance and with a white or yellow or brown color and with the clearest peripheral edges clearly distinguishable from healthy enamel. Sometimes, the defect is so porous that after the eruption, the tooth can break into enamel fractures or cracks, posteruptive enamel breakdown (PEB), under the action of the masticatory forces, leaving the exposed dentin and thus favoring the development of carious lesions.
According to the literature, the world prevalence of different MIH defects varies from 2.4% to 40% based on the different geographical areas; it affects the male sex more and is mainly found on the buccal and occlusal surfaces of the affected elements. Researchers have found that children with MIH require more care than unaffected children: teeth may be more sensitive to food and cold air due to increased porosity and altered structure which also leads to greater penetration of bacteria through the enamel hypomineralized to the dentinal tubules. The increased hypersensitivity, in the most severe cases, hinders the daily brushing maneuvers, causing discomfort and pain.
The pathological picture and the accumulation of plaque for poor hygiene, expose children with MHI to a higher risk of caries, further difficult to treat due to the difficulty in the anesthetization of these teeth hypersensitized by continuous thermal and mechanical stimuli.
The scarcity of the data present in the literature on the treatment of hypersensitivity leads us to evaluate hypotheses of alternative treatments that improve this problem.Last data of 2018 confirm the improvement of the hypersensitivity of the elements with MIH after the use of CPP-ACP based remineralizers compared to those based on fluorine, confirming the data of Ozgul which treated only incisors with MIH. Bekes reports encouraging results after the application of a desensitizing paste based on 8% arginine and calcium carbonate for 8 weeks.
The objective of the study is to evaluate the effectiveness of erosion-infiltration with resin, in permanent molars with MIH, in the attenuation of hypersensitivity during hygiene maneuvers or the intake of cold food / drinks. patients aged 8 to 14 years with sensitivity to thermal stimuli on at least one permanent molar, evaluated with Shiff scale with values above 0 before treatment (T0). The selected elements had no caries or post-eruptive fractures.After a single treatment of erosion-infiltration with ICON, the sensitivity test is carried out at the end of the application (T1), one week after treatment T2, one month and two months later (T3, T4). All reported lower sensitivity values already at the end of treatment with an improvement over time until the total disappearance; 4 continue to have a slight perception of the thermal stimulus at a distance of two months.
The treatment of erosion infiltration with resin ICON is a minimally invasive preventive treatment that allows to occlude, thanks to the fluid resin, the porosity of the enamel affected by MIH significantly improving the problem of hypersensitivity and consequently the maneuvers of oral hygiene at home or the discomfort during taking too cold food and drink. One hypothesis is that the treatment with resin probably improves the irregular structure of the enamel, decreasing mechanical stress on the dental elements and therefore the risk of post-eruptive fractures and the exposure of the tubules to the bacteria.
