Introduzione: La frattura-dislocazione del gomito, denominata triade terribile, comprende tre lesioni anatomiche: frattura del processo coronoideo dell’ulna, frattura del capitello radiale e dislocazione del gomito. L'obiettivo di questa tesi di ricerca è quello di condurre un'analisi dettagliata sugli esiti a lungo termine del trattamento, focalizzandosi sulle complicanze e sul ripristino delle funzionalità, in modo da valutare il livello di ripresa funzionale dei pazienti in funzione di come sono state trattate le singole lesioni. Materiali e metodi: È stato condotto uno studio retrospettivo che ha coinvolto un campione di 35 pazienti trattati presso il Policlinico di Modena nel periodo compreso tra gennaio 2011 e dicembre 2021. I criteri di inclusione sono la contemporanea presenza delle tre lesioni della terribile triade e aver eseguito sia RX che TC. I pazienti sono stati contattati individualmente e sottoposti a due score, MEPS e DASH. Risultati: L’età media del campione è 59 anni e il follow up medio 5,9 anni. Il meps medio mostra un buon livello di funzionalità (87) e il dash medio una percentuale di disabilità residua del 20%. Le complicanze si sono presentate nel 67% dei trattati. Il gruppo di pazienti operati con fissazione del capitello ha un mayo medio del 87% e un dash medio del 18%, il gruppo operato con protesi, un mayo medio del 78% e un dash medio del 30%. Le medie sono risultate comparabili. Le complicanze sono state registrate rispettivamente nel 72% e 75% dei casi. I pazienti con coronoide trattata e non trattata hanno mayo e dash medi equivalenti (mayo medio buono, dash medio inferiore al 25%) Lo stesso è stato riscontrato confrontando il gruppo di pazienti con LCL trattato e non trattato. Conclusione: La terribile triade, ha degli esiti favorevoli, sia in termini di funzionalità che di disabilità residua. Le complicanze maggiori sono molto rare mentre quelle minori sono frequenti. I risultati del trattamento del capitello radiale tramite riduzione o sostituzione sono equivalenti e non si può definire un approccio migliore dell’altro. Se non necessario, non conviene trattare a prescindere il processo coronoideo o il LCL perchè si ottengono esiti funzionali equivalenti a quelli ottenuti nel gruppo dei trattati. Le complicanze minori si presentano frequentemente in tutti i casi, indipendentemente dalla tipologia di intervento effettuata per trattare le singole lesioni. E’ necessario condurre studi più ampi e approfonditi per comprendere pienamente gli approcci migliori da utilizzare nel trattamento delle lesioni della terribile triade.

Abstract
Introduction: The elbow fracture-dislocation, known as the terrible triad, encompasses three anatomical injuries: fracture of the ulnar coronoid process, fracture of the radial capitellum, and dislocation of the elbow. The objective of this research thesis is to conduct a detailed analysis of the long-term outcomes of treatment, focusing on complications and functional restoration, in order to evaluate the level of functional recovery in patients based on how the individual injuries were treated.
Materials and Methods: A retrospective study was conducted involving a sample of 35 patients treated at the Policlinico of Modena between January 2011 and December 2021. The inclusion criteria were the simultaneous presence of the three injuries of the terrible triad and the performance of both X-ray and CT scans. The patients were individually contacted and subjected to two scores, MEPS and DASH.
Results: The average age of the sample was 59 years, and the average follow-up period was 5.9 years. The mean MEPS score indicates a good level of functionality (87), and the mean DASH score shows a residual disability rate of 20%. Complications occurred in 67% of the treated cases. The group of patients treated with capitellum fixation had a mean Mayo score of 87% and a mean DASH score of 18%, while the group treated with prostheses had a mean Mayo score of 78% and a mean DASH score of 30%. The means were found to be comparable. Complications were recorded in 72% and 75% of the cases, respectively. Patients with treated and untreated coronoid process injuries had similar mean Mayo and DASH scores (good mean Mayo score, DASH score below 25%). The same was observed when comparing the group of patients with treated and untreated lateral collateral ligament (LCL) injuries.
Conclusion: The terrible triad has favorable outcomes in terms of both functionality and residual disability. Major complications are very rare, while minor complications are frequent. The results of radial head treatment through reduction or replacement are equivalent, and it is not possible to define one approach as better than the other. If not necessary, it is not advisable to treat the coronoid process or the LCL independently, as functional outcomes equivalent to those achieved in the treated group are obtained. Minor complications frequently occur in all cases, regardless of the type of intervention performed to treat individual injuries. Larger and more in-depth studies are necessary to fully understand the optimal approaches to be used in the treatment of terrible triad injuries.
