FOLLOW-UP A 10 ANNI DOPO TRATTAMENTO ESWL DI CALCOLI RENALI: LA NOSTRA ESPERIENZA
Introduzione
La litotrissia extracorporea ad onde d’urto (ESWL) è entrata ormai da molti anni nella pratica clinica, ma i suoi effetti collaterali a lungo termine sono ancora incerti e dibattuti. Nonostante alcuni autori abbiano trovato un aumentato rischio di diabete ed ipertensione, altri non hanno trovato alcuna correlazione tra queste complicanze ed il trattamento.
Obbiettivo dello studio
Il nostro scopo è quello di trovare l’incidenza dei possibili effetti collaterali a lungo termine ( in particolare ipertensione e diabete ) a dieci anni dal trattamento con un litotrittore elettromagnetico Dornier Lithotripter S XXP.
Materiali e metodi
Al fine di trovare questi dati, i pazienti che hanno avuto un trattamento ESWL di calcoli renali dieic anni fa sono stati invitati a sottoporsi ad un controllo complete per la funzionalità renale, la morfologia renale, la pressione arteriosa e la glicemia. Sono stati anche recuperate tutte le anamnesi cliniche prima e dopo il trattamento. Per comparare in maniera efficace i dati vecchi con quelli nuovi sono stati considerati anche i fattori confondenti, quali colesterolemia, trigliceridemia, indice di massa corporea e l’abitudine tabagica.
Risultati
Cento pazienti con un età attuale di 61 anni (DS: 10,2) sono stati considerati. I trattamenti ESWL sono stati fatti tra il 2002 ed il 2004. Al momento del trattamento, il BMI medio era di 26 (DS:3,9); il 53% dei pazienti erano fumatori, il 35% soffriva di ipercolesterolemia, il 33% era iperteso (il 67% dei quali era in terapia), il 6% era diabetico, la creatininemia media era di 0,96 mg/dl (DS: 0,22) e la VFG media di 92,8 (DS: 17,8). Al controllo, il BMI è di 27,3 (DS: 4,2), i fumatori sono l’11%, il 61% ha ipercolesterolemia, il 39% è iperteso (p=0.37) e sono tutti sotto terapia medica, il 17% è diabetico (p=0.0). La creatininemia media è di 0,92 mg/dl (DS: 0,19) e la VFG media dell’88,1 (DS: 21,7). Il 47% dei pazienti ha ancora calcoli renali a 10 anni dal trattamento.
Discussione
Al controllo dei 10 anni la funzione renale globale, confermata dai valori di creatinina e della VFG non è stata influenzata dall’ESWL. L’aumentata incidenza di ipertensione è coincidente all’incidenza di tale patologia nella popolazione globale (6% in 10 anni).
Conclusioni
Nella nostra esperienza non c’è alcuna evidenza di complicanze a lungo termine legate alla ESWL effettuata con il Dornier Lithotripter S. Questo è uno dei primi report riguardanti la sicurezza a lungo termine dei litotrittori elettromagnetici di terza generazione.

Abstract
10 YEARS FOLLOW-UP AFTER ELECTROMAGNETIC ESWL OF RENAL STONES: OUR EXPERIENCE
Introduction
Many years has passed since extracorporeal shock wave lithotripsy (ESWL) entered into the clinical practice, but its long term side effects are still debated and uncertain. Although some authors suggested an increased risk of diabetes and hypertension, others rejected any relationship between those complications and ESWL.
Aim of the study
Our goal is to assess the incidence of long term side effects (with particular consideration to hypertension and diabetes) ten years after an ESWL treatment performed with an electromagnetic Dornier Lithotripter S XXP.
Materials and methods
In order to achieve this goal, patients who had an ESWL treatment of renal stones ten years ago were invited to undergo a complete medical check-up involving renal function, renal morphology, blood pressure levels and blood glucose assessment. We also recovered all their clinical anamnesis before and after the treatment. We compared past data with the new ones considering also confounding factors like cholesterol and triglycerides values, BMI and cigarette smoke habit.
Results
One hundred patients with a current age of 61 years (DS: 10,2) were considered. ESWL treatments have been performed from 2002 up to 2004. At the time of the treatment, mean BMI value was 26 (DS:3,9); 53% of the patients were smokers, 35% suffered of hypercholesterolemia, 33% had hypertension (67% of them required medical therapy), 6% had diabetes, mean creatinine value was 0,96 mg/dl (DS: 0,22) and mean VFG was 92,8 (DS: 17,8). At the follow-up, BMI was 27,3 (DS: 4,2), smokers were 11%, 61% had hypercholesterolemia, 39% had hypertension (p=0.37) with all those patients undergoing medical therapy, 17% were diabetics (p=0.0). Mean creatinine value at follow up was 0,92 mg/dl (DS: 0,19) and mean VFG was 88,1 (DS: 21,7). 47% of the patients had renal stones at 10 years of follow up.
Discussion
At 10 years follow up, overall renal function assessed as creatinine level and VFG has not been affected by ESWL treatment. The increased rate of hypertension is consistent with the incidence of new diagnosis of such disease in the global population (6% in 10 years).
Conclusions
In our experience, there is no evidence of long term complications after ESWL performed with Dornier Lithotripter S. To our knowledge, this is the first report about long term safety of a third generation electromagnetic lithotripter.
