Introduzione e generalità: La malattia di Parkinson (PD) è la seconda malattia neurodegenerativa più diffusa al mondo. È caratterizzata dalla degenerazione prematura e progressiva dei neuroni dopaminerigici situati nella substantia nigra. Questo comporta una limitazione delle funzioni motorie. Un altro marchio patologico primario della malattia è data dalla presenza dei Corpi di Lewy. Si tratta di aggregati citoplasmatici di una proteina insolubile denominata alfa-sinucleina, che si accumulano nei neuroni dopaminerigici a livello della via nigro-striatale. La malattia di Parkinson presenta due diversi tipi di sintomi; motori e non motori. I sintomi motori sono le principali che caratterizzano la malattia, tra cui bradicinesia, rigidità, tremori, instabilità posturale e lentezza nell’eseguire i movimenti. L’approccio terapeutico primario è quello dopaminergico al fine di compensare la perdita di dopamina e migliorare i sintomi motori del paziente e conferirgli una migliore qualità di vita e maggiore autonomia. Per questo, il gold standard nella terapia del PD è il Levodopa, ma un suo uso a lungo termine porta all’insorgenza della Discinesia indotta da Levodopa (LID), caratterizzata da movimenti involontari. Scopo della Tesi: lo scopo della tesi è quello di indagare quali possono essere i meccanismi alla base della LID e dell’infiammazione in modelli animali di Parkinson generati utilizzando α-syn come specie tossica, per determinare se vi è una maggiore tossicità combinando α-syn e levodopa. Materiali e Metodi: Lo studio ha coinvolto 45 ratti femmine del genere Sprague Dawley e viene sperimentata il modello misto che sfrutta fibrille preformate (PFFs) di α-syn umana e AAV (sovra espressione di α-syn mediata da adenovirus). Entrambi le entità sinucleiniche sono state somministrate a 40 ratti nella zona intra-nigrale, in seguito riceveranno una terapia a base di iniezioni sottocutanee di soluzione salina(0,9%) o levodopa (12 mg/Kg). Il gruppo Naïve (n = 5) serve come gruppo di controllo non subirà né lesione ne riceverà trattamento. Dopo 8 settimane, i ratti sono stati perfusi e il cervello verrà estratto per essere tagliato e ottenere delle sezioni con spessore di 30 µm. Queste verranno sottoposte all’analisi di immunoistochimica (IHC). In questo saggio sono stati utilizzati gli anticorpi TH e FOSB primari e secondari, per determinare la degenerazione cellulare e come marker per possibile LID. La tesi verte sull’analisi istologica del tessuto cerebrale mediante osservazione microscopica e scanner e sulla relativa analisi quantitativa , a cui seguirà l’analisi statistica ANOVA con il software Prism per poi ottenere i dati necessari alla comprensione di questo studio. Risultati: C’è stata una perdita del 40% dei neuroni dopaminergici in seguito alla somministrazione di AAV, che aumenta fino all’80% con l’aggiunta delle fibrille. L’analisi della densità ha dimostrato che la L-dopa non induce una tossicità supplementare rispetto a quella generata da α-syn. Al contrario, risulterebbe essere protettiva nei confronti della tossicità espressa nello striato da parte delle PFFs. Inoltre, L-dopa non aumenta l’espressione di FOSB, che è coerente con l’idea che L-dopa non causi discinesia. Discussione e Conclusione; l’impatto delle PFF nelle cellule α-syn sovra espresse è apparso come potenziamento della tossicità acuta indotta da α-syn. L-dopa potrebbe potenzialmente causare ulteriori danni neuronali, ma ciò non si osserva dai risultati ottenuti in questa tesi. Il modello misto AAV+ PFF è utile per studiare i vari cambiamenti patogenetici e degenerativi della malattia di Parkinson, e fornisce una base promettente per o sviluppo di nuove ricerche. Ma un limite di questo modello è che l’interazione L-dopa e α-syn risulta ancora oggi difficile da osservare in vitro

Abstract
Introduction and generality: Parkinson's disease (PD) is the second most prevalent neurodegenerative disease in the world. It is characterized by premature and progressive degeneration of dopamine neurons located in the substantia nigra. This leads to a limitation of motor functions. Another primary pathological mark of the disease is the presence of Lewy bodies. These are cytoplasmic aggregates of an insoluble protein called alpha-synuclein, which accumulate in dopamine neurons at the level of the nigro-striatal pathway. Parkinson's disease has two different types of symptoms; motor and not motor. Motor symptoms are the main ones that characterize the disease, including bradykinesia, rigidity, tremors, postural instability and slowness in performing movements. The primary therapeutic approach is dopaminergic in order to compensate for the loss of dopamine and improve the patient's motor symptoms and give him a better quality of life and greater autonomy. For this reason, the gold standard in PD therapy is levodopa, but its long-term use leads to the onset of levodopa-induced dyskinesia (LID), characterized by involuntary movements.
Thesis Aim: the aim of the thesis is to investigate what may be the mechanisms underlying LID and inflammation in animal models of Parkinson's generated using α-syn as the toxic species, to determine whether there is enhanced toxicity by combining α-syn and levodopa.
Materials and Methods: The study involved 45 female Sprague Dawley rats and the mixed model exploiting preformed fibrils (PFFs) of human a-syn and AAV delivered overexpression of α-syn overexpression). Both synuclein entities were administered to 40 rats in the intra-nigral zone, after which they will received subcutaneous injections of saline (0,9%) or levodopa (12 mg/Kg) twice daily. The Naive (n = 5) group served as a control with not treatment. After 8 weeks, the rats were perfused, and brain extracted before being cut into 30 µm tick sections for immunohistochemistry (IHC) analysis. In this assay, primary and secondary TH and FOSB antibodies were used to determine cell degeneration and as a marker for possible LID. The thesis focuses on the histological analysis of brain tissue by microscopic observation and scanner and respective quantitative analysis, which will be followed by the ANOVA statistical analysis with the Prism software to then obtain the data necessary for the understanding of this study.
Results: There was a 40% loss of dopaminergic neurons following AAV administration, which increases to 80% with the addition of fibrils. Density analysis showed that L-dopa did not induce additional toxicity compared to that generated by α-syn. On the contrary, protection of fibrils in the striatum suggested that it may be protective against the toxicity expressed in the striatum by PFF's. In addition, L-dopa does not increase FOSB expression, which is consistent with the idea that L-dopa does not cause dyskinesia.
Discussion and Conclusion; the impact of PFF in α-syn cells over-expressed appeared to potentiate acute α-syn-induced toxicity. L-dopa could potentially cause further neuronal damage, but this is not observed from the results obtained in this thesis. The AAV+ PFF mixed model is useful for studying the various pathogenetic and degenerative changes of Parkinson's disease, and provides a promising basis for developing new research. A limitation of this model is that the interaction between L-dopa and α-syn is still difficult to observe in vitro.
