Riassunto analitico
Durante la guerra del Vietnam ci fu un nemico che non apparteneva a nessuno schieramento: la malaria. La malaria uccise più soldati di quanto non abbiano fatto i proiettili, lasciando spesso lesioni neurologiche nei sopravvissuti. Negli anni 70’ ci furono più di 4000 casi nell’esercito statunitense, ma nemmeno i vietcong furono risparmiati. La Repubblica Cinese era alleata del Nord Vietnam così Mao Zedong promosse il progetto 523 che coinvolse più di 500 ricercatori con lo scopo di identificare un farmaco contro questa malattia parassitaria. La scoperta dell’artemisinina è dovuta a Tu Youyou, farmacologa a capo del team che per primo la isolò nel 1972 dall’Artemisia Annua, insignita poi con il premio Nobel nel 2015. L’artemisinina nel tempo ha assunto un notevole significato clinico: le ACT (terapie combinate con artemisinina) sono oggi il trattamento di prima scelta contro la malaria secondo le linee guida della WHO. Oltre alle proprietà antimalariche, evidenze convincenti sono emerse fin dall’inizio degli anni 90’ sulle proprietà antineoplastiche dell’artemisinina e dei suoi derivati in linee cellulari tumorali in vitro, in vivo e in trials clinici di fase I. Questa tesi analizza i probabili meccanismi alla base della citotossicità delle artemisinine. Essi sono controversi poiché il loro effetto è pleiotropico ed include principalmente stress ossidativo, arresto del ciclo cellulare, inibizione dell’angiogenesi e dell’invasività delle cellule cancerose. Le artemisinine sono in grado di alchilare il gruppo eme e le proteine, alterare il potenziale della membrana mitocondriale, interferire con le vie di segnale cancerogene e modulare i recettori steroidei come quello degli estrogeni, dell’androstano e del pregnano. La morte cellulare può avvenire mediante apoptosi, ferroptosi (forma di morte cellulare programmata scoperta di recente), autofagia o (in casi meno comuni) necroptosi. Le artemisinine manifestano potenti sinergie con alcuni chemioterapici d’uso clinico come cisplatino, gemcitabina e rituximab e si prestano come coadiuvanti in una multi-chemioterapia. Aumentano inoltre l’efficacia di radio e fototerapia. Infine è riportata una panoramica su derivati artemisininici dimerici e trimerici dotati di promettente attività citotossica in vitro. Diagnosi precoci e precise combinate ai recenti progressi terapeutici hanno migliorato le probabilità di sopravvivenza dei pazienti neoplastici. Eppure, la moderna medicina è in grado di curare solo due casi di cancro su tre. Grazie alla comprensione della biologia molecolare alla base della carcinogenesi e allo sviluppo nella chimica farmaceutica, sono stati sintetizzati farmaci e sistemi di trasporto più efficaci, come i coniugati farmaco-anticorpo; ma i progressi medici hanno portato ad un aumento esponenziale dei costi. Gli effetti avversi, lo stato di salute precario dei pazienti, la riduzione dei costi, l’eterogeneità delle forme tumorali, le metastasi e la farmaco resistenza sono alcune delle complicanze che rendono la ricerca di chemioterapici sempre attuale. Questa tesi vuole analizzare se le artemisinine possono contribuire ad affrontare questi problemi poiché essendo il loro uso come antimalarici già consolidato e sicuro, potrebbero divenire nuovi antineoplastici economici sia per i paesi sottosviluppati che per i sistemi sanitari dei paesi industrializzati.

Abstract
In later decades many cytotoxic drugs were discovered by ethnopharmacology research and screening substances from the plant, bacteria kingdoms as vinca alkaloids, taxanes, podophyllotoxins and anthracyclines. During the war in Vietnam there was an enemy that did not belong to any deployment: malaria. Malaria killed more soldiers than bullets did and often left the survivors neurologically damaged. In the 70’s there were more of 4000 cases in the US army troops, but even the vietcong were not spared. People’s Republic of China was an ally of north Vietnam so Mao Zedong founded the project 523 (in 23/5/1967): a secret program involving more than 500 researchers with the aim of identifying an effective drug against this parasitic disease. The discovery of artemisinin is due to Tu Youyou, head pharmacologist of the team which first isolated it from Artemisia Annua (in 1972), awarded with the nobel prize in 2015. The value of this award is due to the importance that artemisinin has matured over time: the Artemisinin combination therapies are today the first-line treatment for malaria according to the WHO guidelines.
In addition to antimalarial properties, compelling evidence has emerged since the beginning of the 90’s showing that artemisinin and its derivatives have anticancer activities in a variety of human cancer cell model systems (without toxicity for normal cells) in vitro, in vivo and in early clinical trials. This review analyzes the plausible mechanisms of action of artemisinins and biological pathways involved in their cytotoxicity. Their mechanisms are controversial because the response in cancer cell is pleiotropic and include: arrest of cell cycle, growth inhibition, increased levels of oxidative stress. They are able to shrink xenograft cancer, disrupt the mitochondrial membrane potential and alkylate heme group and proteins. The perturbations of the cancer pathways lead to cell death which can occur via apoptosis, ferroptosis (a recently recognized form of regulated cell death), autophagy or (in few cases) necroptosis. Furthermore, Artemisinins are inhibitors of angiogenesis, cell migration and cancer invasion. They can modulate the steroid receptors as the estrogen, constitutive androstane and pregnane X receptors.
Artemisinins manifest potent synergy with some of the main chemotherapy drugs as cisplatin, gemcitabine and rituximab and this makes them perfect adjuvants for multi-chemotherapy. They also increase the effects of radiotherapy and phototherapy. Finally, this paper providing an overview of the most promising derivatives as dimers and trimers.
Earlier and precise diagnosis, in conjunction with the recent advancements in treatment improve the chances of survival of the patients. Nevertheless, modern medicine techniques are able to cure only two of three diagnosed cancers. Thanks to the advancement in knowledge of molecular biology underlying carcinogenesis and developments in pharmaceutical chemistry, more efficient drugs and drug delivery systems have been synthesized, such as antibody-drug conjugate. Yet, medical progress corresponds to an exponential increase in costs. Therapeutic toxicity, poor individual patient’s health status, cost-minimization, wide heterogeneity among cancers, metastases and multidrug resistance are some of the complications that make the search for new antineoplastic drugs a topical demand. This document serve the purpose to analyze if Artemisinins can deal with these problems since their use is well established, safe and they could be cheap antineoplastic agents for the non-industrialized countries and also for health system.
